      2006 and 2009, please complete the following application.

     Please make a selection. Either the current Executive Director or current Board Chair, is still with the same organization/nonprofit
          that they were a part of when they attended the Leadership Seminar indicated in Question #1.

     Please make a selection. We have submitted the Reflections Web-Form as a part of this application to Alchemy+
          (The Reflections Web-Form must be completed and submitted in order to apply to Alchemy+)

          prior to participation in Session 1 (Peer to Pier) of Alchemy+

     Please make a selection. We commit to familiarizing ourselves with the new Anthology toolkit, and reading the Participants Guide, which will be
          sent to us upon our acceptance into the program.

For the section below we are asking you to indicate your preference in attending the four sessions.

Sessions 1 and 4 are large group activities. Sessions 2 and 3 will be divided into four cohorts of 25 members each.

We ask that you select your preference for either a Friday half-day or Saturday half-day for Sessions 2 and 3.

We cannot guarantee you will get your preferred option, but will work towards accommodating your selection.
